OmniHealth Diagnostics has partnered with Cizzle Bio to expand clinician access to DEX-G2, an innovative blood-based test designed to support the early detection of gastric (stomach) cancer.
DEX-G2 is a minimally invasive biomarker assay performed through a simple blood draw and developed by Cizzle Bio to support earlier detection and clinical decision-making in patients at risk for gastric cancer.
Licensed healthcare providers may begin the onboarding process through OmniHealth Diagnostics by completing the provider intake form below.
How to Start Testing
Step 1 — Download Provider Intake Form
To begin the onboarding process for DEX-G2 testing, please download and complete the OmniHealth Diagnostics Provider Intake Form.
Step 2 — Complete the Form
Fill out all required sections of the intake form. The form is fillable and can be completed electronically.
Step 3 — Submit Your Form
Once completed, please email the form to: customerservice@omnihealthdx.com
Our onboarding team will review your submission and contact your practice regarding next steps for account setup and access to testing.
