Licensed healthcare providers may begin the onboarding process for CIZ1B through OmniHealth Diagnostics by completing the provider intake form below.
How to Start Testing
Step 1 — Download Provider Intake Form
To begin the onboarding process for CIZ1B testing, please download and complete the OmniHealth Diagnostics Provider Intake Form.
Step 2 — Complete the Form
Fill out all required sections of the intake form. The form is fillable and can be completed electronically.
Step 3 — Submit Your Form
Once completed, please email the form to: customerservice@omnihealthdx.com
Our onboarding team will review your submission and contact your practice regarding next steps for account setup and access to testing.
