Kit Instructions

Included: Collection cup or oral fluid device, security seal, biohazard transport bag, requisition form, prepaid return label

 

Steps:

1. Collect specimen following chain-of-custody or standard collection protocols.

2. Secure lid and apply tamper-evident seal.

3. Label specimen with patient name, DOB, and collection date/time.

4. Complete requisition form, noting medications reported by patient.

5. Place specimen in transport bag with paperwork in outer pocket.

6. Return via prepaid courier or approved pickup.
